There are 17 little chests with fragments in the Unnamed City. Chests have different spawn timers from each other, so it's really rare to find them all at the same time. I'm playing on a low pop server where players don't care/know about them so I managed

You need to get the recipes for crafting first though. Find the Archivist and turn them in to the wall behind him. That gives you a random scroll for each fragment.
